The Training Floor – Physical Activity Readiness Questionnaire
Regular physical activity is safe for most people. This questionnaire helps us identify the small number of people for whom activity might not be advisable, or who need to check with their GP first.

Has a doctor ever told you that you have a heart condition and should only perform physical activity recommended by a doctor?
Do you feel pain in your chest during physical activity, or have you experienced chest pain in the past month?
Do you lose your balance because of dizziness, or have you lost consciousness in the last 12 months?
Have you ever been diagnosed with another chronic medical condition (e.g. diabetes, asthma, high blood pressure)?
Are you currently taking any prescribed medication for a chronic condition?
Do you have a bone, joint, or soft tissue problem that could be made worse by exercise?
Is your doctor currently prescribing medication for blood pressure or a heart condition?
Are you pregnant, or have you given birth within the last 6 months?
Is there any other reason you can think of why you should not participate in physical activity?
If you answered YES to any question: Please consult your GP before starting, and inform your trainer/instructor of the details.
