Please read this document carefully. By signing this document, you are giving up certain legal rights, including the right to sue.

This Agreement is entered into between Rocket Ship Enterprises Pte. Ltd., operating under the brand name CrossFit Urban Edge (hereinafter referred to as the "Facility" or "Company"), and the undersigned participant (hereinafter referred to as the "Participant").

1. Assumption of Inherent Risks
I understand that participating in activities at the Facility involves strenuous physical activity, including but not limited to weightlifting, gymnastics, high-intensity interval training, aerobic conditioning, and the use of various fitness equipment.

I acknowledge that these activities carry inherent risks that cannot be entirely eliminated, regardless of the care taken to avoid injuries. These risks include, but are not limited to:

Minor injuries such as scratches, bruises, and sprains.

Major injuries such as joint or back injuries, muscle tears, broken bones, and concussions.

Catastrophic injuries including heart attacks, stroke, rhabdomyolysis, paralysis, or death.

Injuries resulting from the failure of equipment, dropping of weights, or the actions/inactions of other participants.

I voluntarily assume full responsibility for all risks of property damage or physical injury, known or unknown, that may occur during my participation in any programs or use of the Facility.

2. Representation of Physical Fitness
I represent that I am in good physical condition and have no medical, physical, or mental conditions that would prevent my safe participation in these activities. I confirm that I have either received medical clearance from a physician to engage in high-intensity exercise or have voluntarily chosen to participate without such clearance, assuming all related risks. I agree to immediately notify the coaching staff of any changes to my health status or any pain experienced during training.

3. Release of Liability and Waiver of Claims
In consideration for being allowed to participate in activities and use the premises and equipment provided by the Company, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and hold harmless Rocket Ship Enterprises Pte. Ltd., its brand CrossFit Urban Edge, its directors, officers, coaches, employees, independent contractors, volunteers, and agents (collectively, the "Released Parties") from any and all claims, demands, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me. This release includes any claims arising from the ordinary negligence of the Released Parties.

4. Indemnification
I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs, including attorney’s fees, that they may incur due to my presence at the Facility, my use of its equipment, or my participation in any activities, whether caused by my own negligence or the negligence of others.

5. Photo, Video, and Media Release
I understand that the Company occasionally photographs, records, and films classes, events, and training sessions for promotional, educational, and commercial purposes.

By signing below, I grant Rocket Ship Enterprises Pte. Ltd. (trading as CrossFit Urban Edge), its representatives, and employees the irrevocable and unrestricted right to:

Take photographs and video footage of me while participating in activities at or related to the Facility.

Use, publish, distribute, and reproduce these images and recordings in any format (including print, digital, social media, websites, and advertising material) without prior review or approval.

Use these materials without providing me with any financial compensation or royalties.

If you wish to opt-out of the Media Release, you must explicitly notify the Facility manager in writing prior to signing this agreement.

6. Governing Law and Severability
This Agreement shall be governed by and construed in accordance with the laws of Singapore. I agree that if any portion of this Agreement is found to be void, invalid, or unenforceable, the remaining sections shall remain in full force and effect.
