Please complete this form on behalf of your child if they are under 16 years old and joining Strong Kids and Teens Classes.
The PAR-Questionnaire will help you understand whether you should check with your doctor before you or your child starts classes. If you are over 69 years of age and you are not used to being very active, please check with your doctor regardless of your responses. Please read the questions carefully and answer each one honestly, giving explanation if you answer 'yes' to any questions.
If you answered 'yes' to one or more questions, consider talking with your doctor by BEFORE starting classes. You may be able to do any activity you want, as long as you build up gradually and consider any medical advice you have been given, or you may need to restrict activities to those that are safe.
If you answered NO honestly to all PAR-Q questions, you can be reasonably sure that you or your child can safely join classes.

Have you (or has your child) ever had a stroke?
Do you (or does your child) ever have unexplained pains in your chest at rest or during physical exercise?
Do you (or does your child) consistently feel faint or suffer from spells of dizziness?
Do you (or does your child) suffer from asthma and require medication?
Do you (or does your child) suffer from type I or II diabetes?
Do you (or does your child) suffer from any major muscle or joint conditions that may limit you or be aggravated by physical activity?
Do you (or does your child) have any allergies?
Do you (or does your child) Does your child have any learning, behavioural, or sensory conditions that may affect participation?
We like to take pictures during classes for social media and marketing. Do you need to be exempt from these?
I understand that there is a risk of injury associated with participating in activities and using equipment at Newbury CrossFit and Performance Centre, including falls or injury, due to negligence, improper use of equipment, or failure to follow safety instructions.
I agree to indemnify and hold harmless Newbury CrossFit and Performance Centre, its owners, employees, agents, and affiliates from any and all claims, demands, damages, costs, and expenses, including reasonable legal fees, arising out of my use, or my child or dependent's use, of the gym’s facilities and equipment.
I understand that if I attend Open Gym sessions at Newbury CrossFit and Performance Centre, they may not be supervised by a qualified coach, and that certain equipment should not be used whilst alone in Open Gym (peg board and climbing ropes).
