JMI COACHING AND CONSULTANCY LTD - Pre-exercise questionnaire
Please complete this form for yourself if you are over 16 years old and joining an adult CrossFit, Weightlifting or Engine Room class.
Please complete this form on behalf of your child if they are under 16 years old and joining Strong Kids and Teens Classes.
The PAR-Questionnaire will help you understand whether you should check with your doctor before you or your child starts classes. If you are over 69 years of age and you are not used to being very active, please check with your doctor regardless of your responses. Please read the questions carefully and answer each one honestly, giving explanation if you answer 'yes' to any questions.
If you answered 'yes' to one or more questions, consider talking with your doctor by BEFORE starting classes. You may be able to do any activity you want, as long as you build up gradually and consider any medical advice you have been given, or you may need to restrict activities to those that are safe.
If you answered NO honestly to all PAR-Q questions, you can be reasonably sure that you or your child can safely join classes.

Have you (or has your child) ever been told that you have a heart condition or high/low blood pressure?
Have you (or has your child) ever had a stroke?
Do you (or does your child) ever have unexplained pains in your chest at rest or during physical exercise?
Do you (or does your child) consistently feel faint or suffer from spells of dizziness?
Do you (or does your child) suffer from asthma and require medication?
Do you (or does your child) suffer from type I or II diabetes?
Do you (or does your child) suffer from any major muscle or joint conditions that may limit you or be aggravated by physical activity?
Do you (or does your child) have any allergies?
Do you (or does your child) Does your child have any learning, behavioural, or sensory conditions that may affect participation?
We like to take pictures during classes for social media and marketing. Do you need to be exempt from these?

I understand that there is a risk of injury associated with participating in activities and using equipment at Newbury CrossFit and Performance Centre, including falls or injury, due to negligence, improper use of equipment, or failure to follow safety instructions.
I agree to indemnify and hold harmless Newbury CrossFit and Performance Centre, its owners, employees, agents, and affiliates from any and all claims, demands, damages, costs, and expenses, including reasonable legal fees, arising out of my use, or my child or dependent's use, of the gym’s facilities and equipment.
I understand that if I attend Open Gym sessions at Newbury CrossFit and Performance Centre, they may not be supervised by a qualified coach, and that certain equipment should not be used whilst alone in Open Gym (peg board and climbing ropes).

1. Membership and Payments
1.1. Memberships are personal and non-transferable.
1.2. Members must maintain an active payment method on file.
1.3. Failed payments may result in suspension of membership until the balance is cleared.
1.4. Membership prices may change with at least 30 days’ notice.
2. Cancellations and Pauses
2.1. Monthly memberships may be cancelled with two months' notice before the next billing date.
2.2. Memberships may be paused for medical reasons or other circumstances agreed between the owner and member, via written request
3. Class Bookings and Attendance
3.1. Members must book classes in advance via WoDBoard.
3.2. Members must make every effort to cancel bookings at least 2 hours before class start (we accept that this will not always be possible).
3.3. Newbury CrossFit reserves the right to amend class timetables and coaches.
4. Health and Safety
4.1. Members train at their own risk and are responsible for ensuring they are medically fit to participate.
4.2. All new members must complete a PAR-Q (Physical Activity Readiness Questionnaire) before training.
4.3. Members must follow all coach instructions, use equipment properly, and respect health and safety rules.
4.4. Personal belongings are brought into the gym at the owners' own risk.
6. Children and Visitors
6.1. Members must be aged 16 or over unless participating in Strong Kids and Teens with parental consent.
6.2. Children are not permitted on the training floor unless part of a supervised session or agreed by a coach.
6.3. Visitors and children spectating classes must follow coach instructions.
7. Privacy and Data Protection
7.1. Personal data is handled in accordance with UK GDPR and the Data Protection Act 2018.
7.2. We collect and process personal information solely for membership management, class bookings, and communication.
7.3. Members may request access, correction, or deletion of their data at any time.
Newbury CrossFit reserves the right to update these Terms & Conditions at any time. Members will be notified by email or in writing, and continued use of the facility constitutes acceptance of the updated terms.
