RAM Fitness Liability Waiver

I/We agree to participate in a physical fitness regime provided by RAM Fitness, which may include, but not necessarily be reserved to, Functional Fitness training. RAM Fitness has made me fully aware that the fitness regime which RAM Fitness offers, in which I agree to participate, is of a nature that is extremely strenuous and may push me to the limits of my physical abilities. I understand that the regime is not without degrees of risk which may include, but are not reserved to the following:

Injury due to improper use of equipment.
Injury due to a medical condition, whether known or unknown to me.

I willingly assume full responsibility for any and all risks that I am exposing myself to as a result of my participation in the training regime. I hereby certify that I know of no medical problems that could increase my risk of illness and injury as a result of participation in a fitness regime designed by RAM Fitness.

Release of Liability:
In full consideration of the above mentioned risks and hazards and in full consideration of the fact that I am willingly and voluntarily participating in the activities made available by RAM Fitness. I hereby waive, release and discharge RAM Fitness and its employees and volunteers of any kind related to, arising from, or in any way connected with my participation in the fitness regime.

This agreement shall be binding upon me, my successors, representatives, heirs, executors, assigns, or transferees. If any portion of this agreement is held invalid, I agree that the reminder of the agreement shall remain in full legal force and effect. If I am signing on behalf of a minor child, I also give full permission for any person connected with RAM Fitness to administer first aid deemed necessary, and in case of serious illness or injury give permission to call for medical and or surgical care for the child and to transport the child to a medical facility deemed necessary for the well being of the child.

I understand that I am not obligated to perform nor participate in any activity that I do not wish to do, and that it is my right to refuse such participation at any time during my training sessions. I understand that should I feel lightheaded, faint, dizzy, nauseated, or experience pain or discomfort, I am to stop the activity and inform staff. 


RAM Fitness

I agree to allow RAM Fitness, its employees and volunteers to use picture(s), film and/or likenesses of me for advertising purposes. In the event I choose not to allow use of such media I must inform RAM Fitness of this in writing.

Many health benefits are associated with regular exercise, and the completion of PAR-Q is a sensible first step to take if you are planning to increase the amount of physical activity in your life.

For most people physical activity should not pose any problem or hazard. PAR-Q is designed to identify the small number of adults for whom physical activity might be inappropriate or those who should have medical advice concerning the type of activity most suitable for them.

Common sense is the best guide in answering these few questions. Please read them carefully and check the correct answer opposite the question if it applies to you.

RAM Fitness Membership - Contract

Firstly, Welcome to RAM Fitness! We are glad you're starting your fitness journey with us. Please check the below details are correct & please take some time to read our Terms & Conditions.

Your Membership is: Platinum = Unlimited Group Classes a Month, Unlimited Open Gym, Weekly Mobility Class

Terms & Conditions:

1. The Platinum Membership permits the use of the 'RAM Fitness' facility

2. Your membership renews on a Month by Month basis.

3. This contract occurs monthly

4. The contract amount is £88.00

5. Number of Monthly payments is 1.

6. Use of the facility is subject to all rules & regulations in force at the time.

7. The Platinum Membership does not give you priority over any other member or guarantee the availability of classes.

8. The member warrants that he/she/other is in suitable physical health to undertake their chosen activity and knows of no medical or other reason why he/she/other is incapable of engaging in the active or passive exercise and that such exercise would not be detrimental to their health, safety, comfort or physical condition.

9. The member shall not use the facility whilst suffering from any infectious or contagious illness, disease or whilst suffering from other ailments such as open cuts or where there is a risk that use of the facility may be detrimental to the health, safety or comfort of other users.

Cancellation:
10. When cancelling your membership, you must give at least 21 days written notice to RAM Fitness before the 1st of the month. If not given the facility holds the right to keep you for the next contract period.

11. If a payment is missed the facility have the right to take the payment as and when they want.

Class Bookings:
12. Bookings for classes can be made up to 14 days in advance & down to 15 Minutes before the class starts.

13. A minimum of 1 hours notice is required to cancel your class booking.

Conduct:
14. Members must use all facilities & equipment in accordance with advice given out by 'RAM Fitness' staff. Members shall not abuse the equipment & the facilities.

RAM Fitness:
15. Any damage to 'RAM Fitness' property shall be paid for in full by any member who wilfully or negligently causes such damage.

16. Disorderly, rude or offensive behaviour to staff or other users will result in refusal of entry into 'RAM Fitness' facility and may result in termination of the your membership.

17. No Member shall bring intoxicating liquor into the facility or attempt to use the facility whilst under the influence of alcohol, narcotics, or other mood altering substances. Smoking is not permitted within the facility.

18. Members use the facility at their own risk; 'RAM Fitness does not accept responsibility for any harm or injury to any member unless directly caused by employees of 'RAM Fitness'

19. Appropriate clothing & shoes must be worn at all times whilst participating in activities at the facility.

20. 'RAM Fitness' reserves the right to remove, suspend, or ban any users from the facility that are deemed not to be adhering to the facilities rules and regulations.

General:
21. 'RAM Fitness' reserve the right to close the facility or withdraw any activity without notice should it be necessary due to unforeseen circumstances or poor attendance, or to adjust the hours of opening for the purposes of maintenance, decoration, repair or other legitimate business of 'RAM Fitness'. Where possible a minimum of 24 hours of notice will be given.

Data Protection:
By completing this contract you are consenting to 'RAM Fitness' using the information provided on this form in accordance with the 'RAM Fitness Privacy Policy'.
