RAM CrossFit Liability Waiver

I/We agree to participate in a physical fitness regime provided by RAM CrossFit, which may include, but not necessarily be reserved to, CrossFit training. RAM CrossFit has made me fully aware that the fitness regime which RAM CrossFit offers, in which I agree to participate, is of a nature that is extremely strenuous and may push me to the limits of my physical abilities. I understand that the regime is not without degrees of risk which may include, but are not reserved to the following:

Injury to the musculoskeletal and/or cardio respiratory system which can result in serious injury.
Injury due to improper use of equipment.
Injury due to a medical condition, whether known or unknown to me.

I willingly assume full responsibility for any and all risks that I am exposing myself to as a result of my participation in the RAM CrossFit training regime. I hereby certify that I know of no medical problems that could increase my risk of illness and injury as a result of participation in a fitness regime designed by RAM CrossFit.

Release of Liability:
In full consideration of the above mentioned risks and hazards and in full consideration of the fact that I am willingly and voluntarily participating in the activities made available by RAM CrossFit, I hereby waive, release and discharge RAM CrossFit and its employees and volunteers of any kind related to, arising from, or in any way connected with my participation in the fitness regime.

This agreement shall be binding upon me, my successors, representatives, heirs, executors, assigns, or transferees. If any portion of this agreement is held invalid, I agree that the reminder of the agreement shall remain in full legal force and effect. If I am signing on behalf of a minor child, I also give full permission for any person connected with RAM CrossFit to administer first aid deemed necessary, and in case of serious illness or injury give permission to call for medical and or surgical care for the child and to transport the child to a medical facility deemed necessary for the well being of the child.

I recognise that there is risk involved in the types of activities offered by RAM CrossFit.

Therefore, I accept financial responsibility for any injury that I may cause either to myself or to any other participants due to my negligence. I further agree to indemnify and hold harmless RAM CrossFit, their employees and volunteers from liability for the injury of any person(s) and damage to property that may result from my negligent or intentional act or omission while participating in activities offered by RAM CrossFit.

I agree to allow RAM CrossFit, its employees and volunteers to use picture(s), film and/or likenesses of me for advertising purposes. In the event I choose not to allow use of such media I must inform RAM CrossFit of this in writing.

Many health benefits are associated with regular exercise, and the completion of PAR-Q is a sensible first step to take if you are planning to increase the amount of physical activity in your life.

For most people physical activity should not pose any problem or hazard. PAR-Q is designed to identify the small number of adults for whom physical activity might be inappropriate or those who should have medical advice concerning the type of activity most suitable for them.

Common sense is the best guide in answering these few questions. Please read them carefully and check the correct answer opposite the question if it applies to you.

RAM CrossFit Membership - Contract

Firstly, Welcome to RAM CrossFit! We are glad you're starting your fitness journey with us. Please check the below details are correct & please take some time to read our Terms & Conditions.

Your Membership is: Gold = 12 Crossfit Classes a Month, Unlimited Open Gym, Weekly Mobility Class

Terms & Conditions:

1. The Gold Membership permits the use of the 'RAM CrossFit' facility

2. Your membership renews on a Month by Month basis.

3. This contract occurs monthly

4. The contract amount is £60.00

5. Number of Monthly payments is 1.

6. Use of the facility is subject to all rules & regulations in force at the time.

7. The Gold Membership does not give you priority over any other member or guarantee the availability of classes.

8. The member warrants that he/she/other is in suitable physical health to undertake their chosen activity and knows of no medical or other reason why he/she/other is incapable of engaging in the active or passive exercise and that such exercise would not be detrimental to their health, safety, comfort or physical condition.

9. The member shall not use the facility whilst suffering from any infectious or contagious illness, disease or whilst suffering from other ailments such as open cuts or where there is a risk that use of the facility may be detrimental to the health, safety or comfort of other users.

Cancellation:
10. When cancelling your membership, you must give at least 7 days written notice to the Owners before your CONTRACT renewal date. If not given the facility holds the right to keep you for the next contract period.

11. If a payment is missed the facility have the right to take the payment as and when they want.

Class Bookings:
12. Bookings for classes can be made up to 14 days in advance & down to 15 Minutes before the class starts.

13. A minimum of 1 hours notice is required to cancel your class booking.

Conduct:
14. Members must use all facilities & equipment in accordance with advice given out by 'RAM CrossFit' staff. Members shall not abuse the equipment & the facilities.

15. Any damage to 'RAM CrossFit' property shall be paid for in full by any member who wilfully or negligently causes such damage.

16. Disorderly, rude or offensive behaviour to staff or other users will result in refusal of entry into 'RAM CrossFit' facility and may result in termination of the your membership.

17. No Member shall bring intoxicating liquor into the facility or attempt to use the facility whilst under the influence of alcohol, narcotics, or other mood altering substances. Smoking is not permitted within the facility.

18. Members use the facility at their own risk; 'RAM CrossFit' does not accept responsibility for any harm or injury to any member unless directly caused by employees of 'RAM CrossFit'

19. Appropriate clothing & shoes must be worn at all times whilst participating in activities at the facility.

20. 'RAM CrossFit' reserves the right to remove, suspend, or ban any users from the facility that are deemed not to be adhering to the facilities rules and regulations.
General:

21. 'RAM CrossFit' reserve the right to close the facility or withdraw any activity without notice should it be necessary due to unforeseen circumstances or poor attendance, or to adjust the hours of opening for the purposes of maintenance, decoration, repair or other legitimate business of 'RAM CrossFit'. Where possible a minimum of 24 hours of notice will be given.

Data Protection:

By completing this contract you are consenting to 'RAM CrossFit' using the information provided on this form in accordance with the 'RAM CrossFit Privacy Policy'.
