I do hereby assume full responsibility for any and all damages, injuries, or losses that I may sustain or incur, if any, while attending or participating in any exercise programme, sport or physical activity at CrossFit Penzance. I hereby waive all claims against CrossFit Penzance and its employees, for any and all claims for injuries or damages that I might sustain. I understand that there is risk of injury associated with participating in any exercise programme or activity and I certify that I am in good physical condition and have no known disabilities that might otherwise be detrimental to my health or well-being. I certify that all of the information I have provided is correct and true. All applicants must sign.

PHOTO RELEASE
I hereby, give permission for images of myself, captured during regular and special activities, through video, camera and digital camera to be used solely for the purpose of CrossFit or licensed CrossFit affiliate promotional material publications and website, including the CrossFit Penzance Social Media. Last names of under 18s will not be given or posted on the internet or website.

CrossFit Parent & Buggy Class – Terms & Conditions
Overview
This class is designed for parents or caregivers of babies and toddlers (recommended age: 0–24 months) who want to maintain their fitness in a supportive, family-friendly environment. To ensure the safety of all participants, especially the little ones, the following terms and conditions apply:
Age Suitability
* This class is recommended for parents/carers with children aged under 2 years old ( or can be contained within the buggy or pen)
Child Safety Policy
* Babies and toddlers must remain contained throughout the session. This means they must either be:
* Safely secured in a buggy, or
* Inside a designated playpen or secure space provided by the venue.
* Children are not permitted to roam freely in the workout space. This is for their own safety and the safety of all class participants.
* Parents are responsible for ensuring that children are safely and comfortably positioned before the class begins.
Parent Responsibilities
* You must supervise your child(ren) at all times.
* All equipment brought (e.g., buggies, toys) must be clean, safe, and in good working order.
* Please arrive early to set up your space before the class begins.
* If your child becomes unsettled, you are welcome to take a break as needed.
Health & Safety
* Do not attend if you or your child are unwell.
* Ensure your buggy or playpen does not obstruct walkways or pose a tripping hazard.
* All personal belongings and baby gear must be kept tidy and within your designated workout area.
Booking & Cancellations
* Booking in advance is required as spaces are limited.
* Cancellations should be made at least 24 hours in advance where possible.
Instructor’s Rights
* The instructor reserves the right to ask a participant to leave if safety protocols are not followed.
* The instructor may modify class content or structure based on class size, space, or other safety considerations.
Consent
By attending this class, you acknowledge that:
* You understand and accept the terms above.
* You take full responsibility for the supervision and care of your child.
* You release the instructor and venue from any liability related to injury or accidents arising from non-compliance with these safety measures.
