FORTIS Gym Limited - Physical Activity Readiness Questionnaire (PAR-Q)
Regular physical activity has many physical and mental health benefits and is safe for most people. However, some medical conditions or circumstances may require you to seek advice from your GP or healthcare professional before participating in exercise.
All information provided on this form will be treated as strictly confidential and processed securely in accordance with our Privacy Policy.
Please read each question carefully and answer YES or NO honestly. If you answer YES to any question, please provide further details where requested.
Has your doctor ever said that you have a heart condition and that you should only do physical activity recommended by a doctor?
Have you ever felt pain in your chest when you do physical exercise?
In the past month, have you had chest pain when you were not doing physical activity?
Do you often feel faint, have spells of severe dizziness or have lost consciousness?
Have you ever suffered from unusual shortness of breath at rest or with mild exertion?
Has the doctor ever said that you have a bone or joint problem, such as arthritis, that has been aggravated by exercise or that may be made worse by exercise?
Do you have either high or low blood pressure?
Are you currently on any prescribed medicines that may affect your ability to exercise?
Are you currently pregnant, or have you given birth within the last 6 months?
Do you know of any other reason that would affect your ability to participate in physical activity?
FORTIS Gym Limited - Class Booking & Cancellation Policy, Personal Training Cancellation & Scope of Practice Policy and 16-18s Policy
IMPORTANT: Class bookings may be cancelled up to four hours before the scheduled start time through the WodBoard app or desktop version.
IMPORTANT: Late cancellations and no-shows occurring within four hours of the scheduled class start time will be recorded as strikes. Members who accumulate three or more strikes within a calendar month will have a £10 administration fee added to their account. Strike counts reset at the start of each calendar month.
IMPORTANT: No class credits will be lost and no automatic charge will be applied for an individual late cancellation. Members are responsible for managing their own bookings through the WodBoard app or desktop version. FORTIS Gym Limited is unable to cancel bookings on behalf of members via email, WhatsApp, Instagram or any other communication channel. To ensure fairness and consistency for all members, requests to remove strikes will not be accepted.
IMPORTANT: Personal Training session cancellations must be made by 7:00pm on the day before the booked session.
IMPORTANT: To participate in adult classes or Personal Training sessions at FORTIS, you must be 16 years of age or older.

FORTIS Gym Limited - Bumps & Babies | Health Check & Participation Agreement
Our BUMPS & BABIES programme is designed to support you through pregnancy and early motherhood safely, confidently and with the backing of the FORTIS community.
This short form helps us understand where you are in your pregnancy or postnatal journey so that we can coach you safely and provide appropriate exercise adaptations where needed.
By booking and attending a BUMPS & BABIES session at FORTIS, you confirm that you are medically fit to participate in exercise during pregnancy or the postnatal period, or that you have sought appropriate advice from your GP, midwife, health visitor or another suitably qualified healthcare professional where necessary.
You understand that all sessions will be adapted to suit your individual stage of pregnancy or postnatal recovery and that you agree to follow all coaching guidance provided.
You understand that babies are welcome within the training space and that you remain responsible for your baby's supervision, safety and wellbeing at all times.
You acknowledge that sessions take place in a shared gym environment where people are exercising and equipment is being used.
You understand that a designated play area and toys may be provided, although your baby may also remain with you throughout the session.
If you choose to hold or carry your baby while exercising, you do so at your own discretion and accept responsibility for that decision.
FORTIS coaches are responsible for coaching the session and are not responsible for supervising or caring for babies, although they may occasionally provide brief assistance where practical to do so.
You understand that participation in physical activity carries an inherent risk of injury and that you participate voluntarily.
You understand that you should stop exercising immediately and notify a coach if you experience symptoms such as vaginal bleeding, dizziness, chest pain, unusual shortness of breath, calf pain or swelling, persistent abdominal pain or contractions, or fluid leaking.
You understand that FORTIS Gym Limited does not provide medical advice and that all coaching guidance is general in nature. You agree to notify a coach immediately of any changes to your health or symptoms that may affect your ability to exercise safely.

Have you attended your 6-8 week postnatal check and been cleared to return to exercise? (If you have not recently given birth, select No.)
Was your baby delivered by Caesarean section? (If you have not recently given birth, select No.)
Are you currently experiencing pelvic floor symptoms, such as leaking, heaviness or pressure?
Are you currently experiencing pain in your pelvis, lower back or abdomen during movement?
