FORTIS Gym Limited - Physical Activity Readiness Questionnaire (PAR-Q)

Regular physical activity is fun and healthy, and increasingly more people are starting to become more active every day. Being more active is very safe for most people. However, some people should check with their doctor before they start becoming much more physically active.

All information received on this form will be treated as strictly confidential.

Please read the questions below carefully and answer each one honestly: check YES or NO.

If you answered YES to one or more questions, talk to your doctor either in person or by phone before you start becoming more physically active. Tell your doctor what questions you answered yes to on this form. You may still be able to do any activity you want as long as you start slowly and build up gradually or it may be that you need to restrict your activities to those which are safe for you. If you have answered NO to all of the questions, it is reasonably safe for you to participate in physical activity, gradually building up from your current ability level. I have read, understood and accurately completed this questionnaire. In signing, I confirm that I am voluntarily engaging in an acceptable level of exercise, and I fully acknowledge that my participation involves a risk of injury.

FORTIS Gym Limited - Booking/Cancellation, Scope of Practise and 16-18s Policies

IMPORTANT: You can cancel a Class up to 7:00pm the day before a booked Class for the assigned Credit to be automatically returned to your User Account.

IMPORTANT: Personal Training session cancellations must be made up to 7:00pm the day before the booked session.

IMPORTANT: To participate in an adults class or Personal Training session at FORTIS, you must be 16 years or older.
