FORTIS Gym Limited - Physical Activity Readiness Questionnaire (PAR-Q)
Regular physical activity is fun and healthy, and increasingly more people are starting to become more active every day. Being more active is very safe for most people. However, some people should check with their doctor before they start becoming much more physically active.
All information received on this form will be treated as strictly confidential.
Please read the questions below carefully and answer each one honestly: check YES or NO.
Has your doctor ever said that you have a heart condition and that you should only do physical activity recommended by a doctor?
Have you ever felt pain in your chest when you do physical exercise?
In the past month, have you had chest pain when you were not doing physical activity?
Do you often feel faint, have spells of severe dizziness or have lost consciousness?
Have you ever suffered from unusual shortness of breath at rest or with mild exertion?
Has the doctor ever said that you have a bone or joint problem, such as arthritis, that has been aggravated by exercise or that may be made worse by exercise?
Do you have either high or low blood pressure?
Are you currently on any prescribed medicines that may affect your ability to exercise?
Are you pregnant or have you had a baby in the last 6 months?
Do you know of any other reason that would affect your ability to participate in physical activity?
FORTIS Gym Limited - Booking/Cancellation Policy
BOOKING A CLASS:
1. You can check availability and book Classes in advance via the WodBoard app or online. When you book a Class, one Credit will be deducted from your User Account unless otherwise stated (i.e. a free event).
2. We make every effort to ensure that there are a suitable number of Classes available at different times of the day. However, Classes are subject to availability and we do not guarantee that spaces will be available in any given Class, even if you have sufficient Credits in your User Account.
3. If the Class you wish to attend is fully booked, you may choose an alternative Class with remaining spaces. Alternatively, you may join a waiting list for the fully booked Class.
4. If you join the waiting list and a space becomes available more than 24 hours before the Class start time, a Credit will be deducted from your User Account and you will be automatically added to the Class. You will be notified by email and push notification in this event.
5. If you join the waiting list and a space becomes available within 24 hours of the Class start time, you will receive an email and push notification asking you whether or not you’d like to accept the Class space, depending on how much notice you’ve been given before the Class start time. All customers on the waiting list will receive this notification and fastest responder will get the space.
6. If we cancel a Class, your Credit will be automatically refunded to your User Account. We will notify you of this via email, push notification and/or telephone call.
CLASS CANCELLATION:
1. You can cancel a Class up to 7:00pm the day before a booked Class for the assigned Credit to be automatically returned to your User Account.
2. If you cancel a Class after 7:00pm the day before and up until the Class start time you will automatically lose your assigned Credit or be automatically charged £5.00 to your bank account as an UNLIMITED member. This also applies in the event of a Class no-show.
3. If you have a genuine emergency and need to cancel after 7:00pm the day before a booked Class, please inform us via email (info@fortismindbody.com) only and we will determine the result of your late cancellation. All texts, WhatsApp messages and/or messages via any social media platforms will not be accounted for in any circumstance, and your Credit will still be lost or your User Account will still be charged £5.00.
4. A Class or waiting list space can be cancelled either via the WodBoard app or desktop website.
5. The above cancellation policy also applies as standard to any special events or one-off Classes held at FORTIS (i.e. workshops, courses or event days).
FORTIS Gym Limited - Personal Training Cancellation Policy
PERSONAL TRAINING SESSION CANCELLATIONS MUST BE MADE UP TO 7:00PM THE DAY BEFORE THE BOOKED SESSION:
1. If the Client is less than 20 minutes late for a session, the Client must notify the Personal Trainer where possible, and the session will still end at the scheduled time and the Client will be charged at the full session rate.
2. If the Client is more than 20 minutes late for a session, the session will be cancelled and will be charged at the full session rate.
3. If the Personal Trainer is more than 20 minutes late for a session, the Client is owed a complimentary half-hour session that must be redeemed within two weeks.
4. A session can be cancelled up to 7:00pm the day before the scheduled session and this will not result in the session being charged at the full rate and will be rescheduled, subject to availability.
5. In the event of an emergency, if a session were to be cancelled after 7:00pm the day before and up until the session start time it would be down to the Personal Trainer's discretion as to whether or not it is to be charged at the full session rate or rescheduled to a later date.
FORTIS Gym Limited - Scope of Practise
1. The Class Coach/Personal Trainer cannot diagnose and/or prescribe any treatment for injury, disease or other medical problem.
2. The Class Coach/Personal Trainer is not an expert in injury rehabilitation and further medical advice or treatment should be sought elsewhere by the appropriate medical professional, if necessary. Recommendations are available upon request.
3. The Class Coach/Personal Trainer is not a specialist in pre or post-natal training and therefore further medical advice should be sought prior to committing to training.
FORTIS Gym Limited - 16-18s Policy
To participate in an adults class or Personal Training session at FORTIS, you must be 16 years or older. If you are aged between 16 and 18, you should be accompanied by a parent or guardian, who should also intend to participate in the class or session. Under 16s may be able to attend our Teens classes, subject to meeting additional stipulations.
FORTIS Gym Limited - Gym Rules
GYM RULES:
1. Treat everyone with respect.
2. Listen to the guidance given by your coaches: they want what’s best for you.
3. Treat the facility and the equipment like it’s your own.
4. No one should touch any equipment without having a waiver on file at FORTIS.
5. Be accountable for yourself, your actions, your workout, and your safety.
6. Be positive. No negative attitude and no whining in class.
7. Maintain good personal hygiene.
8. Come to class on time and be ready before the class starts.
9. Clean up after yourself after each class or PT session. Put away your equipment and help others. Put away more than you get out.
10. Bring your own water bottle.
11. If you’re finished first on a workout, cheer and motivate others.
12. Stay off the gym floor if you are not working out in the class. Children are permitted to wait in the waiting area, but are the parent/guardian's responsibility and must remain in the waiting area for the full duration of the class. This is for everyone’s safety.
