White Dog Fitness LTD - PAR-Q
If you are between the ages of 15 and 69, the PAR-Q will tell you if you should check with your doctor before you significantly change your physical activity patterns. If you are over 69 years of age and are not used to being very active, check with your doctor.
Please read each question carefully and answer honestly.
If you answered:
YES to one or more of the questions above you should consult your doctor to gain consent before participating in physical activity.
NO to all the questions above it is considered reasonably safe for you to participate in regular physical activity.
Has your doctor ever said you have a heart condition and that you should only do physical activity recommended by a doctor?
Do you feel pain in your chest when you do physical activity?
Do you lose balance because of dizziness, or do you ever lose consciousness?
Do you have a bone or joint problem (for example back, knee or hip) that could be made worse by a change in your physical activity?

Acknowledgment and Assumption of Risk:
I, the undersigned participant, hereby acknowledge that I have voluntarily chosen to participate in fitness and exercise activities provided by Two Tails CrossFit. I understand that these activities may include, but are not limited to, weightlifting, cardiovascular exercise, high-intensity interval training (HIIT), and other physical activities.
I am aware that these activities involve inherent risks, including the risk of personal injury, property damage, or death. I understand that the staff and trainers at Two Tails CrossFit will take all reasonable precautions to ensure my safety, but I acknowledge that accidents and injuries may still occur.
Health and Medical Information:
I certify that I am physically fit and have not been advised otherwise by a qualified medical professional. I understand that it is my responsibility to inform the trainers and staff at Two Tails CrossFit of any pre-existing health conditions, injuries, or medications that may affect my ability to participate in the fitness activities.
I understand that the trainers may modify exercises to accommodate my fitness level and any disclosed health conditions. However, I acknowledge that it is ultimately my responsibility to listen to my body and communicate with the trainers regarding any concerns or limitations.
Informed Consent:
I hereby give my informed consent to participate in the fitness activities provided by Two Tails CrossFit. I understand the nature of these activities and the associated risks. I agree to follow all instructions provided by the trainers and staff and to inform them immediately if I feel any unusual discomfort or pain during the activities.
I understand that Two Tails CrossFit is not responsible for any personal belongings lost or damaged on the premises.
Release of Liability:
In consideration of being allowed to participate in the fitness activities provided by Two Tails CrossFit, I hereby release and discharge Two Tails CrossFit, its owners, trainers, employees, and any affiliated entities from any and all claims, liabilities, damages, or expenses that may arise as a result of my participation in these activities, including but not limited to personal injury or property damage.
I understand that this release includes claims based on negligence, action, or inaction of Two Tails CrossFit, its owners, trainers, or employees.
Photography and Media Release:
I grant Two Tails CrossFit permission to use photographs, videos, or other media taken during my participation in their fitness activities for promotional or educational purposes.

Our Commitment
We are committed to fostering a positive and accommodating environment for all our members. To ensure fairness and smooth operations, we have established the following policies regarding class cancellations, memberships, pauses, and no-shows.
Class Booking & Attendance Policy
• Cancellation Deadline:
o Members must cancel a booked class at least one hour before the scheduled start time to avoid losing their session for that day
o Early Morning Classes: 6:00 AM class cancellations must be made before 10:00 PM the night before
• Class Booking:
Members are encouraged to book classes in advance through the WODBoard app. Classes may be booked up to one week in advance.
• Open Gym Booking:
All Open Gym attendance must be booked in advance via WODBoard for insurance, health & safety, and fire capacity compliance. Open Gym access is only available during designated Open Gym periods and around scheduled class times.
• Class Minimum Attendance:
If no members are booked into a class, it may be cancelled at the gym's discretion one hour before the scheduled start time. Members are encouraged to book in advance to secure their place.
Membership Types & Cancellation Policy
1. Unlimited Membership (Rolling Monthly)
• Access:
Includes access to all standard CrossFit classes and speciality classes included within the current membership offering, such as Olympic Weightlifting, Gymnastics, and Engine sessions (subject to timetable and availability). Any Open Gym access included within a membership is subject to the current Open Gym Policy and booking requirements.
• Billing: Automatically renews each month
• Notice Period: One month’s written notice is required. Cancellation requests must be submitted via the website using the cancellation link located in the footer.
• Cut-Off Date:
To avoid being charged for an additional month, you must submit the cancellation form by the last day of the month before 5:00 PM
• Notice Start:
Your one-month notice will begin on the 1st of the following month, and your membership will end on the final day of that month
• Time-Stamp Rule:
Requests are time-stamped based on submission via the website, not when reviewed by staff
Examples:
Submit form on July 31 before 5:00 PM → Notice starts August 1 → Membership ends August 31
Submit form on July 31 after 5:00 PM → Notice starts September 1 → Membership ends September 30
Submit form on August 1 → Notice starts September 1 → Membership ends September 30
Payments: All payments are non-refundable
2. 10 Sessions Membership (Rolling Monthly)
• Access:
Includes 10 classes per calendar month plus 1 Open Gym session per week, subject to the current Open Gym Policy and booking requirements.
• Unused Classes:
Do not roll over to the following month
• Billing: Automatically renews each month
• Notice Period: Same as Unlimited — one month’s written notice
• Payments: All payments are non-refundable
3. 12-Month Paid In Full Membership (Annual)
• Price: £1275 paid upfront
• Commitment: 12 consecutive months
• Cooling-Off Period:
7 days from purchase, provided the membership has not been used
• Post Cooling-Off Cancellation:
No refunds will be issued once the cooling-off period has expired
• End-of-Term Options:
Prior to the end of your 12-month term, you will be contacted regarding your renewal options.
If no renewal option is selected, your membership will automatically move onto the current monthly unlimited rolling membership at the applicable monthly rate. This rolling membership may be cancelled in line with the standard notice period.
Notice Requirement:
One month’s notice required before end of term
• Notice Start:
Always begins on the 1st of the following month
• Holds:
Pauses are not available on this membership
Examples:
• Contract ends December 31 → Submit form November 30 → Ends December 31
• Contract ends December 31 → Submit form December 5 → Notice starts January 1 → Ends January 31
Membership Plan Changes
• Form Submission:
All membership change requests must be submitted via the official Membership Change Form on the website. The relevant link can be found in the website footer.
• Cut-Off Date:
Requests must be submitted by 5:00 PM on the last day of the month.
Upgrades
• Upgrades to Unlimited Membership (£120/month) take effect immediately
• A pro-rata charge may apply
Downgrades
• Downgrades to the 10-Session Membership (£95/month) require one full calendar months’ notice
• Downgrades take effect from the next billing cycle
General Conditions for All Memberships
• All cancellation requests must be submitted via the website using the relevant link located in the footer
• Requests submitted outside of the official forms may not be processed.
• Cancellations cannot be backdated under any circumstances
• Pausing a membership does not replace or reduce the required cancellation notice period (see Membership Pause section below)
• Returning members will join at the current membership rates applicable at the time of rejoining
• It is the member’s responsibility to ensure all requests are submitted correctly and within the required timeframes
• Failure to follow the correct process may result in delays or requests not being processed
Membership Pause
• Membership pauses are available in 1 or 2-month blocks at £25 per month, with a maximum of 3 months permitted per calendar year
• Pause Request:
Pause requests must be submitted by 5:00 PM on the last day of the month via the Membership Pause Form on the website. The relevant link can be found in the website footer.
• Eligibility:
Membership pauses are available after completing a minimum of 2 full months of membership
• Start Date:
Pauses commence from the 1st day of the following month
• Use During Pause:
If the gym is accessed during a paused period, the membership may be reactivated and normal membership charges resumed
• Cancellation During Pause:
The standard one-month cancellation notice period still applies. Notice periods will begin once the pause period has ended
Example:
Membership paused for September → Cancellation submitted on 20 September → Notice period begins 1 October → Membership ends 31 October
Additional Policies
Changes to Policy:
We reserve the right to amend these Terms & Conditions, policies, membership offerings, pricing, timetable, and services at any time. Members will be notified of any material changes where reasonably practicable.
Transfer of Membership:
Memberships are non-transferable
No Refunds:
All payments are non-refundable under any circumstances
Guests and Visitors:
Guests must purchase a drop-in in advance via WODBoard
Damaged or Lost Articles:
We are not responsible for lost, stolen, or damaged items
Health & Safety:
Members are responsible for ensuring they are medically fit to participate in physical activity. Any injuries, medical conditions, or circumstances that may affect safe participation should be disclosed to the coaching team. Members must follow all reasonable instructions provided by coaches and staff.
Acknowledgement
I confirm that I have read, understood, and agree to the Two Tails CrossFit Wickford Membership Agreement, including policies on cancellations, membership pauses, payments, and guest passes.
I acknowledge that:
• All payments are non-refundable
• All requests must be submitted via the website
• It is my responsibility to remain familiar with the current Terms & Conditions, which may be updated from time to time.
By purchasing a membership, attending classes, booking sessions, or continuing to use the facilities of Two Tails CrossFit, I acknowledge and accept these Terms & Conditions.
