Physical Activity Readiness Questionnaire (PAR-Q)
Regular CrossFit training is fun and healthy. More and more people participate in CrossFit workouts each week.
CrossFit is based upon constantly varied, functional movements, executed at high intensity. It is very safe and we have
CrossFit training for people from age 3 to 90. However, some people should check with their doctor before they start becoming much more physically active.
If you are planning to start doing CrossFit workouts, begin by answering the seven questions in the box below.
The PAR-Q will tell you if you should check with your doctor before you start.
Common sense is your best guide when you answer these questions. Please read the question carefully and answer each one honestly by checking YES or NO.
If you answered yes to one or more questions
Talk with your doctor by phone or in person BEFORE you start CrossFit
Tell your doctor about the PAR-Q and which questions you answered YES.
You may be able to do any activity you want - as long as you start slowly and build up gradually. Or, you may need to restrict your activities to those which are safe for you. Talk with your doctor about the kinds of activities you wish to participate in and follow his/her advice. We’ll scale the workouts to your ability and skill level at all
times, or we may have you sit out if we do not feel that it is safe for you to do the CrossFit workout.
No to all questions
If you answered NO honestly to all PAR-Q questions, you can be reasonably sure that you can:
- Start training using CrossFit methods. Your workouts will be scaled until you reach a level of fitness where your trainers feel you can participate in full CrossFit workouts.
Delay becoming much more active:
If you are not feeling well because of a temporary illness such as cold or a fever - wait until you feel better.
Please note: If your health changes so that you then answer YES to any of the above questions, tell your CrossFit
trainer and ask whether you should change your physical activity plan.
Informed Use of the PAR-Q.
CrossFit Kernow assumes no liability for persons who undertake physical activity, and if in doubt after completing this questionnaire consult your doctor prior to physical activity.
Has your doctor ever said that you have a heart condition and that you should only do physical activity recommended by a doctor?
Do you feel pain in your chest when you do physical activity?
In the past month, have you had chest pain when you were not doing physical activity?
Do you lose your balance because of dizziness or do you ever lose consciousness?
Do you have a bone or joint problem that could be made worse by a change in your physical activity?
Is your doctor currently prescribing drugs (for example, water pills) for your blood pressure or heart condition?
Are you currently taking any prescription medication of any kind?
Do you know of any other reason why you should not do physical activity?
By agreeing to this I wish to participate in the exercise and training program offered by CrossFit Kernow.
I understand there are inherent risks in participating in a program of strenuous exercise. Consequently, I have been examined by a physician of my choice and have obtained his/her approval for my participation in a fitness program within sixty (60) days of the date of creating this profile.
No change has occurred in my physical condition since the date such approval was given which might affect my ability to participate in the fitness program. If a physician has not examined me, I agree to see a physician within sixty (60) days of the date this profile was created to obtain his/her approval for my participation in a fitness program.
I agree that CrossFit Kernow shall not be liable or responsible for any injuries to me resulting from my participation in the fitness program (whether at home, at the training studio, outdoors, or at a corporate, commercial, residential or other fitness facility) or during any of the other services we offer and I expressly release and discharge CrossFit Kernow and its owners, employees, agents and/or assigns, from all claims, actions, judgements and the like which I or my heirs, executors, administrators or assigns may have or claim to have as a result of any injury or other damage which may occur in connection with my participation in the fitness program, excepting only an injury caused by the gross negligence or intentional act of such person or persons.
This Release shall be binding upon my heirs, executors, administrators and assigns.
I understand that I am not obligated to perform nor participate in any activity that I do not wish to do, and that it is my right to refuse such participation at any time during my training sessions.
I understand that should I feel light-headed, faint, dizzy, nauseated, or experience pain or discomfort, I am to stop the activity and inform my Trainer.
I understand the results of any fitness program cannot be guaranteed and my progress depends on my effort and cooperation in and outside of the sessions.
I understand that during a training session, my trainer may have to use Touch Training to correct alignment and/or to focus my concentration on a particular muscle area to be targeted. If I feel uncomfortable or experience any type of discomfort with Touch Training, I will immediately request that my trainer discontinue using this technique.
I understand that the usage of any nutritional supplements is done under my own will.
I understand that CrossFit Kernow may photograph and/or film their client events/sessions and I agree to allow them to use these pictures, films, and/or likenesses of me for promotional purposes.
In the event I choose not to allow the use of the same for said purpose, I agree that I must inform CrossFit Kernow of this in writing (this is not applicable for CrossFit Kids and CrossFit Teens without prior consent by the Parent or Legal Guardian).
If you are a Parent or the legal Guardian of a child taking part in the CrossFit Kids and/or CrossFit Teens training session at CrossFit Kernow you will be agreeing to the information in this liability waiver on behalf of the child taking part.
CrossFit Kernow Terms and Conditions as of November 2023
1 Memberships
Term Memberships: Your first month at CrossFit Kernow will start on the day you sign up.
Term Membership Payments: All term memberships and guest fees are collected via auto-debit (i.e. electronic funds transfer) from either a bank account, debit card or credit card. By providing your account, debit card or credit card information you allow CrossFit Kernow to automatically withdraw fees as they come due on the same day each month.
Membership fees are fixed by CrossFit Kernow and we reserved the right to change these at any time. Members will be notified (via email or notification in wodboard) of any changes in the fees at least 30 days prior to the date of the alteration.
2 Availability of Facility
Members may use the facilities only during scheduled classes, scheduled events or by appointment. Operating hours and current class schedules are listed on our website at www.crossfitkernow.co.uk. CrossFit Kernow reserves the right to change operating hours and/or class schedules at its sole discretion without any effect to this Agreement. CrossFit Kernow endeavours to give members at least 30 days notice with regards to any changes.
If no-one is booked in to open gym 3 hours prior to the session starting, or If no-one is booked into class 1 hour prior to the session starting, CrossFit Kernow have the right to cancel it without notice.
Limited Availability: CrossFit Kernow may close its facility for seminars, competitions, maintenance, selected holidays and other hours based on requirements. CrossFit Kernow may delete, change, discontinue, repair or replace any part or all of the facility without any effect on this Agreement. CrossFit Kernow endeavours to give members at least 30 days notice with regards to any changes.
Limited Use: If you know or should know you have a problem that might prevent you from using CrossFit Kernow (i.e. medical or family emergency, business travel, vacation, etc.) and you sign this agreement; you agree that your membership is limited accordingly. However, because this is your choice, you still must pay your monthly fees as if you could use the facility; there will be no carry-over of monthly fees into subsequent months for any time unspent using the facility in a previous month.
3 Late Payment/Declined Credit Card Fees
A £5 late payment fee will be due and payable for payments received past the 5th day or membership. A declined credit card/debit card will result in a fee of £5. Members who are not current will not be allowed to participate in classes after the 5 days of a failed payment .
4 Cancellation of Membership by Member
A member may cancel his/her membership at any time with at least 7 days’ notice. Please remember partial months are not permitted. If a member cancels on or after the 2nd of any month, then a full payment will still be due the following month on the 1st. There are no refunds for membership fees.
A member may upgrade his/her membership at any point by emailing the staff at crossfitkernow@gmail.com, and his/her membership will take effect immediately, or as requested by the member. A member may downgrade his/her membership with at least a 7 day written notice, and the downgrade will take effect on the first date of the next billing cycle after the first 7 day notice period.
5 Class Bookings
Members must book into classes or open gym using Wodboard. Members are required to cancel any class/open gym they are booked into no less than 12 hours before the start. If a member cancels within this period or does not turn up they will forfeit the session.
6 Class Attendances
If a member has booked a class/open gym but does not attend then the coach will apply a ‘no show’ to the system and the member will lose that session as part of their allowance.
7 Personal Training
All private training sessions booked require 24 hours notice from the member for cancellation to avoid penalty. If a session is cancelled with less than 24 hours notice the full amount of that session will be charged to the member.
8 Lost Property
CrossFit Kernow assumes no responsibility for lost or stolen articles. Lost and found articles not claimed after 30 days will be donated to charity or thrown away.
9 Smoking, Alcohol, Food and Drink
No smoking is allowed in any part of the facility. Members should not use the facilities whilst under the influence of alcohol or drugs. Food or drink may be taken into the facilities if it is in a non-breakable, enclosed container.
10 Supervision of children
Members must be 16 years of age or older unless supervised by a parent or legal guardian at all times, or enrolled in our Kids program.
Guests/Visitors under the age of 16 must be supervised by a parent or legal guardian at all times.
11 Conduct
CrossFit Kernow is committed to the health, safety, welfare of each of its members and staff and will not tolerate unreasonable, threatening, obscene, harassing, indecent or illegal behaviour. CrossFit Kernow has the right to judge behaviour and respond accordingly. This right includes, but is not limited to, termination of membership without refund of any member engaging in unacceptable behaviour.
12 Damages
Members shall pay for any damage to CrossFit Kernow property which results from the wilful or negligent conduct of member, member’s guest or dependent children.
13 Rules and/or Regulations
Members who do not observe CrossFit Kernow’s rules and regulations or who abuse equipment in any fashion will be asked to leave. The management reserves the right to terminate membership of anyone who refuses to observe any of CrossFit Kernow’s rules or regulations. Not all rules and regulations are listed in this agreement. CrossFit Kernow reserves the right to add, change or remove rules, conditions of memberships, opening and closing hours, and all services and facilities offered by CrossFit Kernow.
14 Miscellaneous
These terms and conditions supersede any prior written or oral agreements between them concerning the subject matter of this agreement. The provisions of this may be waived, altered, amended or repealed, in whole or in part, only upon prior written consent of all parties.
