Fortior Fit Ltd - Fortior Fit Athlete Waiver
I (persons who signed this document) understand that my participation in
exercise programmes, classes or events and any related activity conducted by or held by Fortior Fit coaches or instructors is voluntary and at my own risk.
The levels of exercise that I will perform will be at my own pace, based upon my
cardio-respiratory (heart & lung) fitness, muscular strength and endurance.
I hereby state that I will inform the coach or instructor of any symptoms (e.g. fatigue, shortness
of breath, chest discomfort, any pain/discomfort/concern for my safety/benefit) during
my participation in exercise. If I have high blood pressure, diabetes, a heart condition,
an existing injury, recent surgery or if I am taking any prescribed medications that
could affect my performance, I will inform the coach or instructor prior to participating in any exercise.
Fortior Fit will not be liable for any injuries or damage arising
out of participation or use of facilities to the undersigned.
It is agreed that Fortior Fit Coaches and instructors shall not be responsible or liable to the undersigned for
articles lost or stolen whilst making use of the facilities.
I acknowledge that I have read this document in its entirety and understand the
above. I have had the opportunity to ask questions and receive answers.
I understand that I will be given instructions on how to perform an exercise and use equipment and I will ask the coach or instructor any questions if I do not understand.

Please read the following questions carefully and check (X) next to the appropriate answers. Answer all questions honestly and to the best of your ability.

Has your doctor ever said that you have a heart condition (had a stroke, heart attack, or heart surgery)
Has your doctor ever said that you should only do physical activity recommended by a doctor?
Do you feel pain in your chest when you do physical activity?
In the past month, have you had chest pain when you were not doing physical activity?
Do you lose your balance because of dizziness or do you ever lose consciousness?
Have you ever been told by a doctor that you have bone, joint, or muscle problems that could be made worse by physical activity?
Do you have a diagnosed illness that could be made worse by physical activity?
Is your doctor currently prescribing medication for your blood pressure or heart condition?
Do you know of any other reason why you should not do physical activity?

Fortior Fit Ltd - Fortior Fit – Membership Cancellation Policy
Fortior Fit requires a minimum 30-day written notice for all membership cancellations.
This policy ensures adequate time for processing and final billing.
Policy Guidelines:
- Notice must be submitted by email to Info@fortiorfit.com.
- Memberships remain active and billable throughout the 30-day notice period.
- No prorated refunds will be issued for unused time within the final billing cycle.
- Exceptions to the notice requirement may be granted in medical emergencies.
- All cancellations must be confirmed via email by Fortior Fit staff.
- This policy applies to all month-to-month memberships.
Your membership and billing will continue for 30 days after notice is given. No refunds or prorated charges will be issued.
A copy of this policy will be available to all customers in the member hub section of WodBoard.
Fortior Fit Ltd - Fortior Fit – Membership Pause Policy
Fortior Fit requires a minimum 30-day written notice for all pausing of memberships.
This policy ensures adequate time for processing and final billing.
Policy Guidelines:
- Max pause period is one month - if longer than this current membership needs to be cancelled
- A reactivation date must be given for the unpausing of membership
- Notice must be submitted by email to Info@fortiorfit.com.
- Memberships remain active and billable throughout the 30-day notice period.
- No prorated refunds will be issued for unused time within the final billing cycle.
- Exceptions to the notice requirement may be granted in medical emergencies.
- All pausing statuses must be confirmed via email by Fortior Fit staff.
- This policy applies to all month-to-month memberships.
