Please read the waiver below carefully. Your signature and initials acknowledge that you understand and accept all terms and conditions of participation at Apex Fitness LDN.

Assumption of Risk
I understand and acknowledge that participating in physical exercise activities - both supervised and unsupervised - involves inherent risks, including but not limited to the risk of injury, illness, equipment malfunction, and unforeseen incidents.

I voluntarily assume all such risks associated with my use of the gym facilities and equipment, including during open gym hours when no supervision is provided.

Health and Medical History
I declare that I am in good health and have no known medical conditions or physical limitations that would prevent me from participating in strenuous physical activity. I understand it is my responsibility to consult a physician before beginning any fitness program.

Training and Instruction
When attending supervised sessions or classes, I agree to follow all instructions and guidelines provided by Apex Fitness LDN trainers and staff to ensure my safety and the safety of others.

Emergency Medical Treatment
In the event of an emergency, I authorize Apex Fitness LDN and its staff to seek and administer medical treatment as deemed necessary, understanding that any associated costs are my responsibility.

Unsupervised Open Gym Rules and Responsibilities
Open gym access is strictly available to: Full Apex Fitness LDN members, or Individuals who have purchased a valid open gym membership; trial sessions, session packs and Taster Memberships do not qualify for open gym. I understand that without one of the above memberships, I am not permitted to access the gym during open gym hours.

Exercising at Own Risk: I acknowledge that no staff supervision or guidance is provided during open gym hours. I agree to act responsibly and prioritize safety at all times.
Prohibited Activities: I understand that max testing (e.g., 1–5 rep max lifts) is strictly prohibited during open gym hours without supervision.

Equipment Use: I will use all gym equipment properly and safely, returning all items (weights, bars, benches, etc.) to their designated areas after use.
Respect for Others: I will respect fellow members and help maintain a clean, organized, and inclusive workout environment.

Member-Only Access: I understand that open gym access is restricted to active Apex Fitness LDN members only. I will not bring non-members, including friends or family, into the facility.

Code of Conduct
I agree to conduct myself in a respectful and responsible manner at all times while on Apex Fitness LDN premises or participating in its activities.

Consequences for Rule Violations
I understand that violation of any of the above rules may result in:
Immediate suspension or revocation of open gym access
Termination of my membership at the discretion of Apex Fitness LDN management

Release of Liability
I hereby release and discharge Apex Fitness LDN, its owners, employees, trainers, and affiliates from any liability, claims, demands, or causes of action arising from any injury, illness, damage, or loss sustained by me during participation in any gym activities, including both supervised training and unsupervised open gym use.

This waiver applies to all current and future participation in any activities held at or by Apex Fitness LDN.
