RSLT CrossFit Liability Waiver and Release Form

In consideration for being allowed to participate in the activities and programs offered by RSLT CrossFit, I, the undersigned, acknowledge, appreciate, and agree that:

1. Assumption of Risk:
I understand and acknowledge that the activities and exercises offered at RSLT CrossFit include, but are not limited to, weightlifting, cardiovascular exercises, bodyweight exercises, high-intensity interval training, and other fitness activities. I am aware that these activities involve inherent risks, including the risk of serious bodily injury or death.

2. Physical Condition:
I hereby represent that I am in good health and have no physical condition that would prevent my participation in the activities and programs offered at RSLT CrossFit. I acknowledge that I have been advised to consult with a physician prior to and regarding my participation in any exercise program.

3. Release and Waiver:
I, on behalf of myself, my heirs, executors, administrators, successors, and assigns, hereby release, waive, discharge, and covenant not to sue RSLT CrossFit, its owners, officers, employees, and agents from liability for any and all claims, demands, losses, or damages on account of injury, including, but not limited to, personal injury, property damage, or death, caused or alleged to be caused, in whole or in part, by the negligence or other conduct of RSLT CrossFit or its employees or agents, or by any other person using the facilities of RSLT CrossFit.

4. Indemnification:
I agree to indemnify and hold harmless RSLT CrossFit from any loss, liability, damage, or cost they may incur due to my participation in the activities and programs offered at RSLT CrossFit.

5. Photographic Release:
I grant RSLT CrossFit the right to use photographs and/or videos of me taken during my participation in RSLT CrossFit activities for promotional purposes, including but not limited to, social media, website, and other marketing materials.

6. Cancellation

Please note that a 1-month notice is required for membership cancellations. Your membership will stay active for 30 days following your cancellation request, allowing you to continue using all benefits and services during this period. Thank you for your cooperation.

I have read this waiver and release form in its entirety and fully understand its terms. I understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.

Membership Terms & Conditions – RSLT CrossFit

Membership Start Dates
For Monthly Memberships, the first pro-rata payment is collected on the date the Member
signs up to their Monthly Membership package, and Membership begins on that date.
For any fixed-length contracts (e.g., 12 months), the contract commences on the 1st of the
following month. Thereafter, payment is collected on or around the 1st of each month.
All memberships have a minimum contract term and will auto-renew once the fixed contract
term has been served. Examples include:
 12 Month Fixed Membership – contract term of twelve full calendar months.
 Joint 12 Month Fixed Membership – contract term of twelve full calendar months.
 Rolling Monthly Membership – contract term of one full calendar month.
Monthly Memberships enable Members to book into as many classes as they wish on a
monthly basis, subject to availability.

Paid in Full Memberships
For Paid in Full Membership types, full payment is required upfront when the Member signs
up. The Membership will automatically expire at the end of its term. Membership begins on
the date payment is made. Members may renew or switch to a rolling Monthly or fixed-term
Membership afterwards.

Membership Dues & Payments

Membership is payable in advance, either in full or monthly, depending on the Membership
type selected. Monthly fees will be debited on or around the 1st of each month. If the
Member falls into arrears, all outstanding balances must be settled before they can use the
gym.
Monthly Membership fees may be increased at the discretion of and at any time by RSLT
CrossFit, with at least one full calendar month’s written notice. Pricing changes for new
joiners may be made without notice.
Any discounted Membership options are offered on the basis that the Member provides proof
of eligibility. The full Membership fee may be applied until such proof is given.

Cancelling Your Membership

Members on a Monthly Membership contract may cancel with effect only from the end of the
contractual term by giving one full calendar month’s advance written notice, effective from
the 1st of the following month. This should be done by
emailing andrewp@rsltcrossfit.com.
Members cannot cancel before their Membership start date.
Paid in Full Memberships automatically expire at the end of the term unless renewed. No
refunds apply to any portion of fees paid upfront.
Any Member with more than three months remaining on the minimum term may terminate
their Membership at the end of the current month by paying in full 50% of the total future
fees due until the end of the minimum term.

Pausing Your Membership (Freeze Policy)

Members may request to pause their Membership by emailing andrewp@rsltcrossfit.com.
✅ Up to two (2) full calendar months per year may be paused free of charge.
❌ Any additional pause beyond those two months will be charged at £15 per month.
 The minimum freeze period is one full calendar month, starting on the 1st.
 Members must request the pause at least 3 working days before the start date.
 Freeze periods do not count toward the contractual term and will extend the minimum
term by the number of frozen months.
 Memberships cannot be paused during the notice period for cancellation.
 Members may not access the gym during a frozen period using their Membership or
via Drop-in.
 To unfreeze early, a pro-rata fee for the remainder of the current month will apply.
Gym Facilities
Full details of normal opening hours are available upon request at RSLT CrossFit. RSLT
CrossFit reserves the right to vary opening hours, temporarily remove access to equipment, or
close certain areas without notice for cleaning, decorating, repairs, refurbishment, special
functions, or holidays.

Health & Safety

All users must complete a health questionnaire before entry. If any medical conditions are
disclosed, the user must confirm they have taken medical advice or accept responsibility for
exercising without it. Staff are not medically trained and cannot assess physical readiness.
Users are advised to seek medical advice prior to starting any exercise programme if unsure.
The Member warrants upon joining, and on each visit, that they are in good physical
condition and know of no reason they should not engage in exercise, and that such activity is
not detrimental to their health.

Termination of Membership by RSLT CrossFit

RSLT CrossFit may terminate a Membership without notice and with immediate effect if:
 The Member breaches these Terms & Conditions or Gym Rules, repeatedly or via one
serious breach;
 Fees remain unpaid after a request for payment;
 The Member is deemed unsuitable for continued Membership (at RSLT CrossFit’s
discretion);
 The Member puts the health, safety, or well-being of staff, Members or Guests at risk.
Personal Information
Members are responsible for keeping RSLT CrossFit updated with changes to their contact
details. Any written notifications will be sent to the email used at sign-up, or any updated
email provided since. Notices are deemed received two days after sending.
RSLT CrossFit takes the protection of Members’ personal information seriously and will only
contact Members about services and information relevant to the gym.
RSLT CrossFit is unlikely to accept a new Membership application from someone whose
Membership was terminated under the above clause.

Changes to Terms & Conditions

RSLT CrossFit may amend these Terms & Conditions at any time, including introducing
additional terms. Members will be notified, and changes will take immediate effect.
Contract Terms
A completed Application Form, these Terms & Conditions, and the waiver together form a
binding Membership contract with RSLT CrossFit. Members should read all documents in
full before signing up.
Failure by RSLT CrossFit to enforce any rights at any time shall not constitute a waiver of those rights. Failure to act upon a breach does not imply such behaviour is acceptable. Except where permitted by contract, neither RSLT CrossFit nor the Member may alter the terms without the express agreement of the other.
