Informed Consent Waiver:
I understand and acknowledge that participation in CrossFit training, Open Gym activities at CrossFit MTS, and related exercises at CrossFit MTS involves significant physical exertion and carries inherent risks. These risks include, but are not limited to, muscle strains, sprains, fractures, joint injuries, heart-related issues, dehydration, or other serious medical events.
I acknowledge that:
1. Inherent Risks: CrossFit and Open Gym, and related exercises at CrossFit MTS activities involve complex and intensive movements. These risks may not be entirely eliminated, even with proper supervision and equipment.
2. Physical Condition: I affirm that I am physically fit and free from any medical conditions that may impair my ability to participate safely. I have consulted a physician before engaging in this program if necessary.
3. Supervision and Guidance: While instruction and supervision may be provided, I am responsible for my own conduct and safety, including following instructions and using equipment correctly.
I realise I have the right to and should discontinue any test or exercise at any time due to excessive discomfort related to dizziness, lightheadedness, muscle or joint pain, chest pain, difficulty in breathing, or any other reason. I also assume responsibility to report immediately any of these previous conditions.
CrossFit is deliberately, and by design, a real world, functional fitness programme. By this we mean that some of the exercises and techniques that we use can be dangerous if performed incorrectly or with poor technique. This is why we spend a great deal of time and energy on drilling technique and skills through the foundations program. This sort of exercise regime is not without risk, though.
Furthermore, I am aware that it is my responsibility to receive clearance from my physician prior to participation in vigorous activity and that any previous or current health-related problems or injuries will be disclosed.
Lastly, I understand it is my responsibility to eat correctly before and after workouts and to stay hydrated throughout the day. I will bring a water bottle and towel to all workouts, and wear the appropriate training attire and shoes for the workout.
Release of Liability and Waiver:
In consideration of being permitted to participate in activities at CrossFit MTS, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby:
1. Waive, release, and discharge CrossFit MTS, its owners, employees, trainers, and representatives from any and all claims, liabilities, demands, and causes of action arising out of or connected with my participation, whether caused by negligence or otherwise.
2. Indemnify and hold harmless CrossFit MTS and its affiliates from any costs, expenses, or damages related to injuries or other harm resulting from my participation.
3. Consent to emergency treatment as necessary if an injury occurs.
Participant Agreement:
I have read this form completely and have been fully informed of the risks associated with performance testing and/or training. I realise that all participation in training and/or testing is voluntary and by signing this document hereby freely consent to participate.
Social media photography release:
Consent to Photograph, Record, and Use Images for Social Media Purpose:
This consent form grants CrossFit MTS the right to photograph, record, and use images and videos of members for promotional and social media purposes, including but not limited to platforms such as Facebook, Instagram, Twitter, and the gym's official website.
1. Authorization
I, the undersigned, hereby authorize CrossFit MTS and its representatives to:
Take photographs and/or video recordings of me while participating in activities at CrossFit MTS.
Use, edit, and publish these photographs and recordings on CrossFit MTS’s social media accounts, promotional materials, and website.
2. Usage Rights
I understand that:
The photographs and recordings may be used for promotional, educational, and marketing purposes.
My name and personal information may be used only with my explicit consent.
No compensation or royalties will be provided for the use of my images or recordings.
3. Ownership
All photographs, recordings, and related materials shall remain the property of CrossFit MTS. I waive any right to inspect or approve the finished product or promotional material where my likeness appears.
4. Release of Liability
I release and discharge CrossFit MTS, its staff, affiliates, and representatives from any and all claims, liabilities, and demands arising out of the use of the photographs or recordings, including claims for defamation, invasion of privacy, or rights of publicity.
5. Duration of Consent
This consent remains valid indefinitely unless I provide written notice to revoke it.
6. Opt-Out Clause
I understand I have the right to refuse consent without any impact on my membership or participation in gym activities.
CrossFit MTS Open Gym:
1. What is Open Gym?
Open Gym provides a flexible training environment where you can work on your fitness goals independently, outside of structured class times. Whether you want to focus on strength, skills, or conditioning, the gym is here for you.
2. Eligibility for Open Gym
• Completion of the CrossFit Foundations session (if new to CrossFit).
• Familiarity with core CrossFit movements and equipment.
• Approval from a coach or staff member.
• Signed waiver and induction acknowledgment.
3. Hours of Operation
• Open Gym hours are listed on WoDBoard.
• Note: Open Gym hours may overlap with class times. Priority will be given to structured classes. During class time there are dedicated areas that can be used for open gym, but space is limited.
4. Safety Guidelines
General Rules
• Warm-Up: Always warm up before starting your workout.
• Spotters: Use a spotter when attempting heavy lifts.
• Weights: Use safety clips/collars for all barbell lifts.
• Lifting Area: Perform Olympic lifts in designated areas only.
• Equipment Usage: Return equipment to its original location after use.
• Cleanliness: Sanitize equipment after each use.
In Case of Emergency
• Report injuries immediately to staff or call the emergency services using the dedicated call point at the gym (located at reception).
• First aid kits are available at reception.
• AEDs are located outside the building next to the entrance door.
5. Gym Etiquette
• Respect Others: Be mindful of noise levels and avoid unnecessary distractions.
• Audio/Music: Headphones are to be used, speakers at the gym can only be used for class.
• Equipment Priority: Yield equipment to class participants during overlaps.
• Personal Belongings: Store bags and personal items in designated areas.
• No Drop Zone: Only drop weights in approved areas.
6. Workout Guidelines
• Plan Your Session: Have a workout plan to make the most of your time.
• Scaling: Modify movements appropriately for your fitness level.
• Avoid Overtraining: Rest adequately between sets and sessions.
7. Supervision and Coaching
• Open Gym is not supervised. However, staff members may be available for questions.
• Private coaching is available upon request for additional fees.
8. Prohibited Activities
• Horseplay or reckless behavior.
• Unauthorized coaching of other members.
• Use of the gym outside approved hours.
9. Equipment Usage Policy
• Inspect equipment for damage before use.
• Report any malfunctioning or damaged equipment to staff.
• Do not remove equipment from the premises.
10. Terms and Conditions
• Membership must be active to access Open Gym.
• Non-transferable and non-refundable.
• Gym policies may be updated at management's discretion.
