**Authority to consent**
I confirm that I am the child's parent or legal guardian, or that I otherwise have lawful authority to give the permissions and information set out in this agreement. I will tell GRAFT promptly if my authority changes or if another person with parental responsibility raises a concern about the child's participation.
**Nature of the activity**
I understand that HYROX Young Stars is a coached children's fitness course that may include age-appropriate running, jumping, carrying, pushing, pulling, bodyweight exercise, resistance exercise, games and use of fitness equipment. Activities will be adapted according to the child's age, experience and apparent ability.
**Understanding and acceptance of risk**
I understand that physical activity cannot be entirely free from risk. Possible risks include slips, trips, falls, collisions, muscle soreness, strains, sprains and other accidental injury. More serious injury or illness is possible but uncommon. I consent to the child taking part with this understanding.
I understand that this acceptance of the ordinary risks of physical activity does not remove GRAFT's responsibility to take reasonable care and does not exclude or restrict any liability that cannot lawfully be excluded or restricted.
**Health information and fitness to participate**
I confirm that I have provided accurate and complete information about the child's health, medication, allergies, injuries, disabilities, additional support needs and any other matter that may affect safe participation. I will update GRAFT before the next session if anything changes.
Where I am uncertain whether participation is suitable, I will seek advice from an appropriate healthcare professional. I understand that GRAFT coaches do not diagnose medical conditions or provide medical clearance.
Coaching, adaptation and stopping activity
I authorise GRAFT coaches to adapt, pause or stop the child's activity where they reasonably consider this necessary for safety, wellbeing, behaviour, inclusion or effective coaching. I understand that GRAFT may ask for further information or medical guidance before allowing participation where a disclosed issue requires clarification.
**First aid and emergencies**
If the child becomes unwell or is injured, I authorise appropriately trained GRAFT staff to provide reasonable first aid and to contact the emergency services where they reasonably believe this is necessary. GRAFT will take reasonable steps to contact me or the emergency contact provided. This does not authorise non-emergency medical treatment beyond first aid.
**Behaviour and safety expectations**
I will ensure the child understands that they must follow reasonable coach instructions, use equipment only as directed, behave respectfully, and tell a coach if they feel unwell, unsafe or in pain. GRAFT may remove a child from an activity or session where behaviour creates a safety or wellbeing concern.
**Arrival, collection and authorised adults**
I remain responsible for ensuring that the child is delivered to and collected from the agreed location at the correct times. I will provide GRAFT with the names of adults authorised to collect the child and will notify GRAFT in advance of any change. The child must not leave alone unless GRAFT has expressly agreed a written arrangement with the parent/guardian in advance.
**Personal property**
I understand that personal possessions are brought to the facility at the owner's risk, except where loss or damage results from a matter for which GRAFT is legally responsible.
**Liability and legal rights**
Nothing in this agreement excludes or restricts liability for death or personal injury caused by negligence, fraud or fraudulent misrepresentation, breach of statutory duty, or any other liability that cannot lawfully be excluded or restricted. Nothing in this agreement affects the child's or parent/guardian's statutory rights.
**Declaration**
I have read and understood this agreement. I confirm that the information I have provided is accurate and that I consent to the child's participation in HYROX Young Stars subject to these terms.
