Participant Health Declaration, Assumption of Risk and Waiver

By joining and participating in activities at Legion CrossFit Maidenhead, operated by LCFM Limited, I acknowledge and agree to the following:

I confirm that I am participating voluntarily and that I believe I am in good physical health and medically fit to undertake CrossFit and other high-intensity functional fitness activities.

I confirm that I have disclosed any medical condition, injury or illness that may affect my ability to participate safely and understand that it is my responsibility to seek appropriate medical advice before participating if I have any concerns regarding my health or fitness.

I understand that CrossFit and associated activities involve strenuous physical exercise, including but not limited to weightlifting, gymnastics, running, rowing, jumping, climbing, use of resistance equipment and other functional movements. I acknowledge that these activities carry inherent risks, including muscle strains, sprains, cuts, bruises, joint injuries, fractures, heat-related illness, serious injury and, in rare circumstances, death.

I understand these risks and voluntarily choose to participate.

I agree to exercise within my own capabilities, to follow all coaching instructions, to use equipment only as directed and to immediately inform a coach if I feel unwell, experience unusual pain or become aware of any issue that may make continued participation unsafe.

I understand that I am responsible for exercising within my own limits and for making coaches aware of any injury, illness or medical condition that may affect my participation. I accept responsibility for my own decision to participate and for my own safety while training.

I acknowledge that while Legion CrossFit Maidenhead, LCFM Limited, its directors, coaches, employees and volunteers will take reasonable care to provide a safe training environment, it is not possible to eliminate all risks associated with physical exercise.

To the fullest extent permitted by law, I agree not to hold LCFM Limited, trading as Legion CrossFit Maidenhead, its directors, coaches, employees or volunteers liable for any injury, loss or damage arising from the ordinary risks inherent in participation in CrossFit and fitness activities.

Nothing in this agreement excludes or limits liability where such liability cannot legally be excluded under the laws of England and Wales, including liability for death or personal injury resulting from negligence.

I confirm that I have read and understood this declaration, that I have had the opportunity to ask questions before participating and that I agree to be bound by its terms.

I also consent to Legion CrossFit Maidenhead taking photographs or video footage during classes and events and using such images for promotional, marketing and social media purposes unless I notify the gym in writing that I do not wish to be included.
