Jamie George Fitness Ltd - Movement Mumbles - Terms and Contract

This Training Terms and Contract is made and entered into by and between Jamie George Fitness Ltd and yourself.

Trainer and Client are sometimes collectively referred to in this agreement as the parties.
The parties hereby agree to the following terms and conditions.

1. General Terms

1a) Client acknowledges that he/she/they is agreeing for the service of coaching provided by Movement Mumbles working under Jamie George Fitness Ltd.
1b) Trainer will design and provide appropriate program for the Client that reflects the Client’s objectives, fitness level and experience.
1c) Trainer will design and provide appropriate program for the gym classes that reflects the members objectives, fitness level and experience.
1d) Each training session will be given a 60minute time slot, unless stated by Trainer.
1e) Open gym sessions will not be under the guidence of Trainer.
1f) It is the responsibility of the Client to correctly use the facility and equipment for its intended use and do so at their own risk.
1g) The facility is not responsible for the loss, theft or damage to any personal belongings brought onto the premesis.

2. Health & Wellbeing

2a) It is the responsibility of the Client to ensure they are fit, well and healthy to partake in any fitness sessions provided by the Trainer.
2b) All Clients that have any underlying health issues or injuries are to inform the Trainer before sessions begin.
2c) It is the Client’s responsibility to inform the Trainer of any new health issues or injuries that may occur during a training session.
2d) Training sessions are undertaken at the risk of the Client.
2e) We advise Clients to seek medical advice before using our Spa and Recovery suite.

3. Payments and Memberships

3a) Payments for training sessions/memberships to be made before delivery.
3b) Clients memberships to be a minimum of three months and then give one months’ notice of contract termination from use of facility to the Trainer.
3c) Cancellation Policy applies to all Movement Mumbles Clients. See below.

4. Cancellation Policy

4a) Classes can be rearranged up to 1 hour before the start time by the Client or the Trainer.
4b) Personal Training sessions cancelled within 24 hours of the start time by the Client will be lost.
4c) Personal Training sessions cancelled within 24 hours of the start time by the Trainer will be rearranged.

5. Spa and Recovery

Hot and Cold Therapy Suite.

If you have any of the following conditions, please seek medical advice before using our facility:
- Cardiovascular Disease
- Circulatory or Vascular Disease
- Diabetes
- Skin Conditions
- Nueropathy or reduced sensation
- Recent Injury, fracture or surgery
- DVT or Blood clots
- Cancer (Past or Present)
- Fever Infection or Flu-like symptoms
- Respiratory conditions
- Pregnancy
- Epilepsy or Seizures
- Metal implants or Pacemakers
- Allergies to chlorine

The policies above were developed with the goal of creating a mutually respectful relationship between the Client and the Trainer.

I have listed my 'Medical/Injury Details' to my full knowledge.

Medical/Injury Details

I agree that Movement Mumbles Gym employees shall not be liable for any loss, damage or injury that I may sustain of my participation in any activity or use of the facilities.

I agree to follow all instructions and safety guidelines provided by gym staff.

I accept full responsibility for any injuries or harm caused by my failure to follow safety rules.

I consent to the use of photographs or video of me taken at Movement Mumbles for promotional purposes.

I confirm that I have read, understood, and agree to the above policy from Movement Mumbles. I acknowledge the risks involved in physical activity and voluntarily assume full responsibility for any injuries or damages that may occur while using the gym facilities. I confirm I am in good health and capable of participating in physical exercise. By accepting this document, I release the gym and it's staff from any liability arising from use of the facility.
