ATHLETE WAIVER
● I understand that participating in any aspect of fitness inside the gym is potentially hazardous and that I should not enter and participate unless I am medically able and properly trained.
● I understand that activities may include but are not limited to: power lifting, gymnastics, Olympic lifting, running, rope climbing, jumping, pulling, throwing and load carrying. I understand that such activities are likely to be an intense test of my cardiovascular system.
● I will not take part in any classes that I am aware are beyond my abilities. If I am aware of, or remotely concerned, that I have a medical condition that might interfere with me participating safely, I should seek professional medical advice and withdraw from the class.
● Below I have disclosed and given details of all personal medical conditions/special medical needs that could influence my performance throughout the competition. ..................................................................................................................................................... ...........................................................................................................................................................
● I accept the rules, instructions and warning notices that have been presented to me. I accept that all classes carries its own risks and I will not carry out any activities (or the class in its entirety) if for any reason I feel that it is not suitable for me.
● I will notify the Medical team and the coaches immediately if I feel unwell before, during or after the event.
● I hereby grant full permission to CrossFit Shearwater to use any photographs, videotapes, motion pictures, website images, recordings or any other record of this event.
● I acknowledge that CrossFit Shearwater accepts no responsibility for loss or damage to property or possessions before, during or after the event.
● By signing below, I accept all of the information presented above and am happy to proceed/participate in all classes in the gym on my membership.
I agree to assume all of the foregoing risks and accept full responsibility for my own damages following such injury, permanent disability, or death.
I release, waive, and discharge, and agree not to sue Crossfit Shearwater, the venue of the classes and it's owner, any parent company and any subsidiaries and all its respective agents, affiliates, associates, officers, directors, owners, and employees (collectively "Releases") from demands, losses, damages on account of any bodily injury, death or property damage caused or alleged to be caused while or in part by Releases or any other party’s actions, inaction's, or in part by my actions.
I consent to emergency medical care and transportation in order to obtain treatment in the event CrossFit Shearwater deems appropriate. This Release extends to any liability arising out of or in anyway connected with the medical treatment and transportation provided in the event of an emergency.
I have read the Waiver, Release, and Indemnity Agreement and understand that by signing the below, I have given up substantial rights.
