I hereby acknowledge and agree to the following terms and conditions:
1. I have voluntarily chosen to participate in physical exercise and activities at Focus Hybrid Ltd (hereinafter referred to as "the gym"). I understand the nature of these activities and recognise that they carry inherent risks, including but not limited to personal injury, property damage, and medical issues.
2. I acknowledge that I am in good health and do not suffer from any condition that would preclude my participation in the gym's activities. I am responsible for consulting with a medical professional before engaging in any activities if I have concerns regarding my health. I must also inform the gym of any changes regarding my health that may impact my ability to safely participate in the gym's activities.
3. I hereby waive and release Focus Hybrid Ltd, its owners, employees and affiliates from any and all claims, liabilities, or demands arising from my participation in any activities, including any injuries, damages, or losses incurred during my participation.
4. I agree to indemnify and hold harmless Focus Hybrid Ltd for any claims made by any third party in connection with my participation in the gym's activities.
5. I understand that this waiver and release is binding upon my heirs, assigns and legal representatives.
6. I acknowledge that I have read this waiver in its entirety, understand its contents, and voluntarily agree to its terms.
