Fort47 Ltd - Fort47 Member Agreement & Risk Disclosure

1. Assumption of Risk
I understand that participating in CrossFit and fitness training at Fort47 involves inherent risks, including but not limited to: slips, falls, collisions, and physical exertion. I acknowledge that I am voluntarily engaging in these activities and accept full responsibility for any injury or damages resulting from participation.
2. Release of Liability
I release and discharge Fort47, its owners, coaches, employees, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to my participation in gym activities, including negligence to the fullest extent permitted by UK law.
3. Health and Medical Disclosure
I confirm that I am in good physical condition and have no medical or mental condition that would prevent me from safely participating. I take full responsibility for my health and any consequences arising from my participation.
4. Emergency Care Consent
If I require emergency medical treatment, I authorise Fort47 staff to seek appropriate care on my behalf. I understand that I am responsible for any associated costs.
5. Gym Rules and Conduct
I agree to follow all gym policies, safety guidelines, and instructions provided by Fort47 coaches. I understand that failure to do so may result in temporary or permanent suspension of membership.
6. Photo and Video Release
I grant permission for Fort47 to use images or videos of me for promotional purposes, unless I notify the gym in writing.
7. Minors
If signing for a minor, I confirm I am the parent or legal guardian and accept all terms on their behalf.
8. Governing Law
This agreement is governed by the laws of England and Wales and any disputes will be handled under their jurisdiction.
