Assumption of Risk
I, the undersigned, understand that participation in physical training activities at or under the direction of CrossFit 1864 involves inherent risks. These risks include, but are not limited to: slips or falls; muscle strains or sprains; injury from equipment; and potential for serious injury or, in rare cases, death.

I confirm that I am voluntarily choosing to participate and fully accept and assume all risks, whether caused by my own actions, the actions of others, or the condition of the premises or equipment used.

I confirm that I have disclosed any relevant medical conditions and that I am fit and able to participate in physical exercise.

Release of Liability
In consideration for being allowed to participate in activities at CrossFit 1864, I agree that CrossFit 1864, its staff, coaches, employees, and agents are not responsible for any personal injury, loss, or damage to me or my property, unless caused by their proven negligence.

I understand that this waiver does not exclude or limit liability for death or personal injury caused by negligence, as per the Unfair Contract Terms Act 1977.

Indemnity
I accept responsibility for any injury I may cause to myself or others through my own negligence. I agree to reimburse CrossFit 1864 for any legal costs incurred in connection with enforcing this agreement.

I also agree to indemnify CrossFit 1864 from any liability for injury, death, or property damage caused by my negligent or intentional acts while participating in any gym activities—whether on-site, off-site (e.g. parks, outdoor sessions), or during travel related to gym events.

Medical Treatment Consent (For Minors)
If I am signing on behalf of a participant under the age of 18, I confirm that I am the parent or legal guardian and give full consent for their participation.

I authorise CrossFit 1864 staff to administer first aid if required. In the event of a serious medical emergency, I give permission for emergency services to be contacted and the child to be transported to a medical facility.

Use of Photography and Video (GDPR Consent)
I understand that photographs or videos may be taken during training sessions or events. I give my consent for these to be used by CrossFit 1864 for marketing or promotional purposes, including social media, website, or printed materials.
I understand I can withdraw this consent at any time by contacting CrossFit 1864.

Acknowledgement
I have read, understood, and voluntarily agree to the terms of this waiver. I acknowledge that by signing this form, I am giving up certain legal rights.

I, the undersigned, confirm that I have read and understood the CrossFit 1864 Class Registration Policy. I understand that there are important rules regarding class booking and cancellations via TeamUp, and failure to follow these may result in penalty charges.

Penalty System
Late Cancellation

I must cancel my class at least 2 hours before it starts to avoid penalty.

If I cancel after the cut-off, it is considered a Late Cancellation.

I am allowed one late cancellation per month. If I have a second within 30 days, a £20 fee will be charged to my TeamUp account.

Early Morning Classes:

6:00 AM classes must be cancelled by 9:00 PM the night before.

7:00 AM classes must be cancelled by 10:00 PM the night before.

No Show

If I fail to cancel and do not attend, this will be considered a No Show.

No Shows incur an automatic £20 penalty fee on my TeamUp account.

These policies have been in effect since 1st July 2021.

For Parents/Guardians of Minors
If signing on behalf of a minor, I acknowledge and accept that these rules apply to them as well. I give permission for CrossFit 1864 to apply any necessary penalty fees in line with the policy.
