IMPORTANT: PLEASE READ CAREFULLY BEFORE SIGNING. THIS IS A LEGAL DOCUMENT.
By signing this waiver, you acknowledge and agree to the following terms and conditions for participation in fitness classes, including but not limited to Hyrox training sessions, hosted at Musclemorph Hybrid.

1. ASSUMPTION OF RISK
I understand that participation in fitness activities, including Hyrox training, involves inherent risks such as, but not limited to: muscle
strains, sprains, fractures, cardiovascular events, and other physical injuries. I voluntarily choose to participate, fully aware of the risks involved, and I accept full responsibility for my physical and mental well-being during and after participation.

2. MEDICAL CLEARANCE
I confirm that I am physically and medically fit to participate in strenuous physical activity. I have not been advised otherwise by a
qualified medical professional. I understand that I am solely responsible for consulting with a doctor prior to beginning any exercise program.

3. WAIVER OF LIABILITY
I hereby release, discharge, and hold harmless Musclemorph Hybrid, its owners, coaches, instructors, employees, agents, and
affiliated entities from any and all liability, claims, demands, or causes of action that may arise from personal injury, illness, property damage, or death sustained while participating in or around the facility.

4. INDEMNIFICATION
I agree to indemnify and hold harmless Musclemorph Hybrid and all associated parties against any and all claims brought by or on behalf of me, including attorney’s fees and court costs related to such claims.

5. PHOTO / VIDEO RELEASE
I grant permission to Musclemorph Hybrid to use photographs and/or video footage taken during classes for promotional purposes. I understand I may opt out by notifying staff in writing.

6. EMERGENCY MEDICAL TREATMENT
In the event of an emergency, I authorize Musclemorph Hybrid staff to secure medical treatment as deemed necessary. I understand that I am financially responsible for any such treatment.

7. ACKNOWLEDGMENT OF UNDERSTANDING
I have read this waiver thoroughly and understand its terms. I voluntarily agree to all conditions, and by signing below, I waive certain legal rights, including the right to sue.
