The PARQ has been designed to identify the small number of people for whom it would be wise to seek medical advice before starting. Please read each question carefully and answer every question honestly.

2. Do you have chest pain bought on by physical activity?
3. Do you tend to lose consciousness or fall over as a result of dizziness?
4. Has your doctor ever recommended medication for your blood pressure or heart condition?
5. In the past month, have you had any chest pain when you were not doing any physical activity?
