Photography/Video Release
Participants involved in any activities offered by Unity Wellness Limited may be photographed or videotaped during training. The undersigned hereby consents to the use of these photographs and/or videos without compensation, on the Unity Wellness Limited website or in any editorial, promotional or advertising material produced and/or published by Unity Wellness Limited.
Unity Wellness Limited accepts no responsibility for loss or damage to personal property within the studio, car park or other areas of the facility. Members are responsible for their own property and understand that they bring personal items to studio, car park and other areas of the facility at their own risk. Internal and external CCTV is used for the purpose of safety against theft, safety of our members and the prevention of crime. The cameras record based on movement and the footage is held for up to 30 days
Waiver and Release of Liability
I hereby acknowledge that use of the studio facilities and equipment, participation in the classes, and in the training provided at Unity Wellness Limited by Unity Wellness Limited exposes me to inherent risks which can include accidents, falls, illness, injury or even death.
I confirm that I know of no medical, physical or mental reasons why I would not be capable of performing the physical activity in which I choose to participate at Unity. I acknowledge my responsibility in communicating to a staff member prior to commencing activity, any physical and/or mental concerns affecting me which could conflict with participation in my chosen studio activities.
I assume all risks of injuries associated with use of Unity Wellness Limited and its facilities including, but not limited to falls, contact with other participants, exercises and techniques taught to me by trainers employed by or providing services for Unity Wellness Limited and all other risks being known and appreciated by me.
Having read this waiver and knowing these facts, in consideration of accepting my use of the studio facilities and of services provided to me at Unity Wellness Limited by Unity Wellness Limited I agree to Hold Harmless, Waive and Release Unity Wellness Limited, its directors, employees, representatives, anyone else providing a service at Unity Wellness Limited and successors from any responsibility, liabilities, demands or claims of any kind arising out of my use of facilities at Unity Wellness Limited or out of my participation in training, classes or anything else organised by or on behalf of Unity Wellness Limited.
