This form must be completed before taking part in classes at Illustrious Fitness. It helps us ensure that training is safe and appropriate for you. Please answer all questions honestly and inform a coach if your health status changes.

Exercise Waiver & Participant Agreement

I understand that participation in physical exercise and fitness training involves a risk of injury. I voluntarily agree to participate in training sessions, classes, and activities at Illustrious Fitness and assume all risks associated with exercise.
I confirm that the information provided in this questionnaire is accurate to the best of my knowledge and that I am physically able to participate in exercise. I agree to inform Illustrious Fitness staff of any changes to my health that may affect my ability to train safely.
I release Illustrious Fitness, its coaches, and staff from liability for injuries or conditions that may arise from participation in exercise activities, except where caused by negligence.

Member Declaration

Full Membership Agreement
This agreement is between:
Illustrious Fitness.
And
The Member (as detailed in the registration form)
1. Membership Term
1.1 All memberships are subject to a minimum term of three (3) months from the start date.
1.2 Following completion of the 3-month minimum term, membership continues on a rolling monthly basis.
1.3 After the minimum term, cancellation requires 30 days’ written notice.
2. Fees & Payment
2.1 Membership fees are payable monthly in advance via approved recurring payment method.
2.2 Payments must be made on the agreed billing date.
2.3 Failure to make payment may result in:
Immediate suspension of access
Recovery of outstanding fees
Referral to debt recovery
3. Cancellation Policy (Strict)
3.1 Cancellation requests must be submitted in writing to: ian@illustriousfitness.com
3.2 The 3-month minimum term must be completed before cancellation can take effect.
3.3 A minimum of 30 days’ written notice is required after the minimum term.
3.4 If a Member cancels payment without notice:
The contract remains active.
Fees for the notice period remain payable.
Illustrious Fitness reserves the right to recover unpaid amounts.
4. Early Termination
Early termination during the 3-month minimum term is only permitted in the following circumstances:
Relocation more than 15 miles away (proof required)
Medical condition preventing exercise (doctor’s note required)
Approval is at management discretion.
5. Freezing Membership
5.1 Membership may be frozen for medical reasons (minimum 3 weeks, maximum 3 months per year).
5.2 Written request and medical evidence required.
6. Health Declaration
Members confirm they are physically fit to participate in:
HIIT
Bootcamp
Boxercise / Pad Work
Strength & Conditioning
Participation is at the Member’s own risk.
7. Code of Conduct
Illustrious Fitness operates a zero-tolerance policy for:
Abuse toward staff or members
Unsafe equipment use
Disruptive behaviour
Membership may be terminated without refund for serious breaches.
8. Liability
Nothing excludes liability for death or personal injury caused by negligence.
Personal belongings remain at the Member’s own risk.
9. Data Protection
Data is processed in accordance with UK GDPR.
10. Governing Law
This agreement is governed by the laws of England and Wales.
