Illustrious Fitness Ltd - Member Health Questionnaire (PAR-Q) & Exercise Waiver

This form must be completed before taking part in classes at Illustrious Fitness. It helps us ensure that training is safe and appropriate for you. Please answer all questions honestly and inform a coach if your health status changes.

Exercise Waiver & Participant Agreement

I understand that participation in physical exercise and fitness training involves a risk of injury. I voluntarily agree to participate in training sessions, classes, and activities at Illustrious Fitness and assume all risks associated with exercise.
I confirm that the information provided in this questionnaire is accurate to the best of my knowledge and that I am physically able to participate in exercise. I agree to inform Illustrious Fitness staff of any changes to my health that may affect my ability to train safely.
I release Illustrious Fitness, its coaches, and staff from liability for injuries or conditions that may arise from participation in exercise activities, except where caused by negligence.
